Weight Loss Challenge
The Weight Loss Challenge is a 20-week Biggest
Loser style friendly competition that challenges all
community employers and social groups to join and
see whose team can safely lose the largest
percentage of bodyweight!

By signing up a team, you have the chance to
maintain the healthy weight you've always wanted
and win the highly-coveted Golden Scale traveling
trophy in the process-- proclaiming your
commitment to attaining "Healthy Weight for Life!"

The guidelines are simple and flexible. Plus, we'll
provide the educational tools to help you succeed!

First weigh-in: Week of January 20-24
All teams form their own incentive program and agree to program guidelines to foster
fairness and safe weight loss. Individuals submit registration forms to chosen Team
Captains by Friday, January 10 2014. Team registration deadline is Thursday,
January 16 2014. Team Captain training wil be offered to all organizations.

Weight Loss Challenge
Steps: How to Join
Joining the Challenge is easy! For more specific details,
consult the program guidelines in this packet.

1 Check with your group
Is there interest in your organization/social circle to form a team? Who
would be your "team captain" to organize your group's participation? Is
there a place & digital scale where everyone can weigh in on the first
and last weeks? Make sure everyone can weigh in on the same day for
the first, mid and last weigh-ins.

2 Decide your involvement

Who/how many people will be in your team? Optional: Do you want to
hold weekly weigh-ins for your team to recognize individuals' weekly
efforts in weight loss? If so, what rewards do you want to give them?

Send in your forms

Team Captain distributes Individual Regist. Form to team members, and
submits completed Individual and Team Regist. Form to CCH (see
information below) by January 16 2014.

Weight Loss Challenge
Team Registration Form
All Elmore County-area groups (busineses, non-profits, social groups) are invited to compete! Please list the participants on your
team. If your team is comprised of more than one organization or social group, please list them accordingly in the spaces
provided below. Every participant listed below must also submit an individual registration form to participate. All documentation is
sent to the contact info listed at the bottom of this form.
First weigh-in: January __, 2014 Last weigh-in: June __, 2014
First weigh-in must take place on the week beginning January 20th 2014. Last weigh-in must take place on the same day (i.e., a
Tuesday) as the first weigh-in, during the week beginning June 2nd 2014. Each team decides on a specific weigh-in day for both
the first and last weeks of the competition.
Team Captain:
Team Captain is in charge of setting up weigh-ins, recruiting team members,
setting up rewards for weekly competition if your organization chooses to hold
them, and recording & reporting info (such as first & last weigh in #'s)

Although not required, each team is strongly encouraged to set up their own pool for weekly

This form is to be submitted to your Team Captain no later than Friday January 10 2014. This form must be
received by the Center for Community Health in order for your Weight Loss Challenge participation to be valid.

Program Guidelines


Starting date: Week beginning 1/20/2014
Midway point: Week beginning 3/31/2014
Ending date: Week beginning 6/2/2014

Rules:
Teams may be comprised of one or more businesses, organizations, or social groups.
The Weight Loss Challenge Winner will be the team who loses the largest % of body weight during the
competition. (The team's total number of pounds from first weigh-in will be compared to total number of pounds
at last weigh-in.) The winning team receives the "Golden Scale" travelling trophy, which is kept by the winning
team for public display until the next Weight Loss Challenge.
Each team must weigh in on the first and last weeks (week of Jan. 20 and week of June 2). Teams can choose
whichever day in that week which works best for them, but both weigh-ins must be on same day of week: if you
weighed in on a Wednesday of first week, you must weigh in on Wednesday of last week. All participants in the
team must weigh in on the same scale. You must use a digital scale for weigh-ins.
Individuals submit registration forms to their Team Captain by Friday, January 10 2014; Team Captains are
responsible for submitting Team Registration Form and all Individual Registration Forms for their team by
Thursday, January 16 2014. Submit forms to the Center for Community Health (CCH) via: email
wiersmab@slhs.org, fax 580-2683, or P.O. Box 1270 Mountain Home, ID 83647.
Team Captains also must send in your team's total number of pounds to (CCH) on the first week, midway point
week and the last week of the competition. After the last week, CCH will perform the math and report the
competition's winning team. The midway point figure is required but will serve only to report progress made.
If participants from your team stop weighing in, their last recorded weight must be included in the figure you
send us for the midway point and final weigh-ins. In other words, when you figure your total number of pounds
for your team, you must include the latest figures from those who `dropped out.'
Each person is to choose their own eating and exercising program. However, tools will be provided to assist you,
such as informational emails.
You are not allowed to use prescription medications designed for rapid weight loss. Participants are also not
allowed to use HCG.
You will not be allowed to have elective surgical procedures to lose weight during the competition - If you get
sick and have to have an emergent procedure that is not done specifically to lose weight we will not throw you
out of the competition.... But we hope everyone stays healthy.
Each team sets up their own weigh-ins. Participants are asked to wear approximately the same type of clothing
each weigh in; must remove jackets/shoes. No cell phones/radios on person when weighing in.
This is a friendly competition, please be supportive of your fellow participants!

Suggestions:
In addition to the mandatory weigh-ins on the first, midway point and last week of the competition, each team
may choose to organize their own weekly weigh-ins to recognize individual efforts in their team. If so, Team
Captains are encouraged to set up a pool and accept money from participants, or help locate rewards for the
team members who lose the most weight each week, etc.
For organizations, you may want to look at forming internal teams to compete against each other. For instance,
if you have 10 team members, you might form Team A and Team B for mini-challenges. This information would
not be submitted to CCH and would be for the purpose of adding fun to your organization's competition.
You can request the Weight Loss Challenge Tracking Spreadsheet by emailing wiersmab@slhs.org to help track
individuals' weekly weigh-ins. The Excel spreadsheet will produce useful information (such as % of bodyweight
lost) for you automatically. Consider communicating this info to individuals for motivational purposes!
You can total your team's individual weigh-ins to get your team's total weight. Any reliable household digital
scale should get the job done! You can have weigh-ins over the course of your team's chosen day.
Don't forget to communicate information about weekly weigh-ins (if any) and incentives to your team members!
