Who is eligible for the Pfizer MAINTAIN Program?Eligibility requirements of the new program include:• Loss of employment since January 1, 2009• Prescribed and taking a Pfizer medicine for at least 3 months prior to becoming unemployedand enrolling in the program• Lack of prescription drug coverage• Can attest to financial hardship ii. How does this program work?Eligible individuals who are in need of help paying for their existing Pfizer prescriptions will beasked to complete an application form and submit it to Pfizer throughwww.PfizerHelpfulAnswers.com or by calling 1-866-706-2400. The application form asks thatpatients provide proof of unemployment and requests an attestation of financial need. OncePfizer has confirmed that the applicant meets the program’s requirements, Pfizer will send a 90-day supply of the patient’s Pfizer medicine directly to his or her home.People who qualify for this program will receive their Pfizer medicines for free for up to 12 monthsor until they become re-insured (whichever comes first). More than 70 Pfizer medicines arecovered through the new program. If assistance is still required after one year, patients can applyto Pfizer’s other patient assistance programs, which provide medicines for free or at a savings atthe doctor’s office, community health centers, federally-qualified hospitals and at the pharmacy.If the applicant does not meet the new program’s requirements, we will determine whether he orshe would be eligible for Pfizer’s other patient assistance programs. Pfizer offers the largest andmost extensive patient assistance programs in the United States, enabling qualifying individualswho lack prescription coverage and need a Pfizer medicine to get help. iii. When does the program begin? And is there an end date?The program will be fully operational on July 1, 2009, but people who need immediate assistancecan call 1-866-706-2400 to get help. The program applies to eligible Americans who havebecome unemployed since January 1, 2009 and is expected to be open for enrollment throughDecember 31, 2009. At that time, it will be re-evaluated to determine if the program will berenewed.iv . Are dependent spouses, domestic partners (covered adults) and children eligible for thisprogram?Yes. If family members were taking Pfizer medicines for three months before unemployment andbefore enrolling in the program, they too can be helped. They will need to complete a separateapplication form.v .Which medicines are available through this program?More than 70 Pfizer primary care medicines will be available through the program. Patients mayqualify for other patient assistance programs, including RSVP for specialty, FirstResource foroncology, and Pfizer Bridge Program for endocrine care medicines.v i. How do people get help if they became unemployed before January 1, 2009?Pfizer runs the largest and most extensive family of patient assistance programs in the UnitedStates. People who became unemployed before January 1, 2009, and need help paying for aPfizer medicine, should contact Pfizer Helpful Answers at 1 to find out which of our other patientassistance programs can help. For example:• Connection to Care™, Sharing the Care® and Pfizer Hospital Partnership Program helppeople with low income get free Pfizer medicine through their doctors’ offices, and atcommunity health centers and federally-qualified hospitals.• Our Pfizer Pfriends™ program helps people of any income get savings on Pfizermedicine.People who need a Pfizer medicine that do not have insurance for prescription medicines shouldcall our toll-free number) or visit our Web site (www.PfizerHelpfulAnswers.com) to find out if theyqualify for free Pfizer medicines, or savings on Pfizer medicines.v ii. Why is Pfizer launching the MAINTAIN Program?Nearly 46 million Americans lack health insurance coverage, and that number is increasing as joblosses have escalated. Pfizer wanted to do what it could to help ease the stress associated withsimultaneously losing one’s job and one’s health insurance. This program helps ensure thatanyone in the United States who has recently become unemployed and lost their insurance, andwho needs help paying for their Pfizer medicine, can continue to get it.v iii. How did Pfizer come up with this the new program?The inspiration for the new program was generated by Pfizer employees who were witnessingfriends, family and neighbors struggle to make ends meet after losing their jobs. Employeesproposed the idea of the MAINTAIN Program to Pfizer’s senior leadership team last month. Thespeed with which we created and approved this program is proof of our commitment to helppeople access the medicines they need to live healthier lives. Pfizer has been helping uninsuredAmericans get access to our medicines for more than 20 years. ix. How are Pfizer employees involved in this program?The inspiration for the new program was generated by Pfizer employees who were witnessingfriends, family and neighbors struggle to make ends meet after losing their jobs. As part of theproposal, employees wanted to be able to do their part. Donations are voluntary and will bematched by the Pfizer Foundation according to established guidelines. x. What will happen if someone is prescribed a Pfizer medicine after they lose their job? Willthey qualify for the new program?If someone who needs a Pfizer medicine loses their job, and with it their health insurance, theother Pfizer patient assistance programs may be able to help them. If someone is prescribed aPfizer medicine after they lose their job, they should call our toll-free number (866-706-2400) orvisit our Web site (www.PfizerHelpfulAnswers.com) to apply for one of our other patientassistance programs. xi. How does this program differ from Pfizer’s other patient assistance programs?This is a new patient assistance program designed to provide Pfizer medicines for free to recentlyunemployed and uninsured individuals and their families so that they can continue anuninterrupted course of therapy. Applicants will not be required to enroll through a doctor’s office,as is the case with our other programs, as they will already have a Pfizer prescription in hand. Inaddition, rather than verifying income, we will only ask that applicants attest to financial need andprovide proof of unemployment. We are doing this to make it as easy as possible for people whohave found themselves in a stressful financial situation. xii. Will Wyeth medicines be part of this program?Pfizer and Wyeth continue to operate as two separate companies prior to the completion of theproposed acquisition. xiii. Is Pfizer planning a similar program for people outside the United States?We help people all over the world get Pfizer medicines because we believe that part of ourresponsibility as a company is to help those in need get access to our medicines. We continuallyreview and evolve our philanthropic activities to ensure that they are helping meet current needs.xiv .Is this program available in Puerto Rico?No. The program is not available in Puerto Rico, the U.S. Virgin Islands or Guam.About Pfizer Helpful Answersxv . What is Pfizer Helpful Answers? Why does Pfizer do this?There are 46 million people in the United States who do not have health insurance to help pay forhealth care, such as prescription medicines, that can help them prevent and manage disease. AtPfizer, we believe that all Americans should have access to quality, affordable health coverage.Pfizer has been committed – for more than two decades – to making it easier for people withoutinsurance to get the Pfizer medicines prescribed by their doctors. In 2004, Pfizer launched PfizerHelpful Answers, a family of assistance programs for the uninsured and underinsured who needhelp getting Pfizer medicines. It is the largest and most extensive patient assistance program inthe U.S. In just the past five years (2004-2008), Pfizer patient assistance programs helped 5.1million patients get 51.1 million Pfizer prescriptions for free or at a savings – valued at $4.8 billion.Pfizer Helpful Answers is a joint program of Pfizer Inc and the Pfizer Patient AssistanceFoundation™.xv i. What type of programs does Pfizer offer?There are three main types of programs under the Pfizer Helpful Answers umbrella: Programsthat offer Pfizer medicines for free; programs that offer savings on Pfizer medicines; and,programs that provide free medicines or reimbursement services for Pfizer specialty products:Pfizer’s free medicine programs include:• Connection to Care, which provides free Pfizer medicines via doctors’ offices;• Sharing the Care, which provides free Pfizer medicines via federally-funded communityhealth centers; and,• Pfizer’s Hospital Partnership Program, which offers free Pfizer medicines via hospitalsthat serve a disproportionate share of uninsured patients.To be eligible for these programs, patients cannot have prescription coverage and must haveincomes at or below 200% of the Federal Poverty Level, adjusted for family size (e.g., $21,660 fora single person, $44,100 for a 4 person household, etc. in 2009).Pfizer’s medicine savings programs: Eligible patients can enroll in Pfizer Pfriends, which offerssavings on Pfizer medicines right at the pharmacy counter. The only eligibility requirement is thatpatients not have prescription coverage. There are NO income limits.Pfizer’s specialty programs: Pfizer runs three programs for its specialty products. Theseprograms offer reimbursement services for patients with insurance. The services include benefitsverification, help with prior authorizations if required, help with appeals if required. For peoplewithout coverage, these programs also provide free medicines if income eligibility guidelines aremet. The programs are:• FirstResource for oncology products;• RSVP for anti-infectives and HIV/AIDS products; and,• Pfizer Bridge Program for growth hormone products.Industry programs: In addition to the Pfizer-specific programs, Pfizer participates in industryprograms, such as Together Rx Access, which is sponsored by several pharmaceuticalcompanies, and offers savings on a broad range of medicines right at the pharmacy counter.Through Together Rx, most cardholders save between 25 and 40 percent on brand-nameprescription medicines. Pfizer also participates in the Partnership for Prescription Assistance, orPPA, a gateway – sponsored by the pharmaceutical industry – that provides a single point ofaccess to more than 475 public and private patient assistance programs, including Pfizer HelpfulAnswers.xv ii. Have you seen an increase in enrollment to Pfizer’s existing patient assistance programs?We recognize that many people and families are feeling the impact of the economic downturnthrough the loss of jobs and healthcare benefits. For that we reason, we announced on March 19,2009 that Together Rx Access®, a prescription savings program sponsored by Pfizer along withseveral other pharmaceutical companies, has expanded the program’s eligibility income levels tohelp even more uninsured individuals and families save on the medicines they need to stayhealthy and to manage chronic conditions.
